Steps to begin occupational therapy with Farfalla Integrative Health
1. Have your doctor send us a prescription.
- You’ll ask your doctor’s office to fax us a prescription for occupational therapy. (See insurance and prescription information sections below). Our fax number is 866-284-0293.
2. Schedule your initial session and complete paperwork online before that date.
- Once we receive the prescription, we’ll contact you to schedule your first visit.
- We’ll also send you links to fill out necessary forms online. You’ll complete and electronically sign the forms before your first session. You can always contact us if you have questions about the forms.
- Services can take place in-person at the office in Lafayette Hill, PA or virtually via telehealth. On-site visits (e.g., at your home or workplace) may be possible depending on your location. (Additional travel fees may apply to on-site visits and would be discussed prior to scheduling.)
3. Attend your first session.
- The first session is an initial evaluation. We’ll assess what’s working and what’s not, specific concerns you have, and what you hope to accomplish. We’ll also complete appropriate tests and outcome measures during this visit.
- Together, we’ll come up with a plan with timeframes to help you meet your goals.
- We may begin treatment on the first session if time allows.
4. Attend follow-up sessions according to the planned schedule.
- Follow-up sessions include customized treatment interventions based on your needs and the plan we established.
- At each session, we check to make sure we’re staying on track toward your goals. If your goals change, we’ll update the treatment plan as needed.
- When you’ve either met your goals or are ready to stop therapy, we’ll do a discharge assessment during our last session and make sure you have a plan for going forward.
Insurance
Insurance information
Farfalla Integrative Health is an out-of-network provider and does not submit claims to any insurance company.
Payment for occupational therapy services is required in full prior to the start of each session.
Superbills (i.e., detailed receipts) can be provided after each visit for you to submit to your insurance company for potential reimbursement if you choose.
If you have Medicare or Medicaid:
- Because of current federal law restrictions, Farfalla Integrative Health cannot provide occupational therapy services to you even if you are willing to pay out of pocket.
If you have a Flexible Spending Account (FSA) or a Health Savings Account (HSA):
Occupational therapy services are eligible for reimbursement from flexible spending accounts (FSA) and health savings accounts (HSA). A letter of medical necessity from your doctor may be needed. Check with the organization that manages your HSA or FSA for details.
If you have private insurance (i.e., any plan that is NOT Medicare or Medicaid):
- You may be eligible for some reimbursement if you have out-of-network benefits on your insurance policy. This worksheet can help guide your conversation with the insurance company about your policy’s details and any reimbursement requirements.
- There is no guarantee of reimbursement as coverage depends on your insurance plan.
- Full payment for services is required upfront even if you plan to submit to your insurance company for out-of-network reimbursement.

Who needs a prescription?
Pennsylvania residents must have a prescription from your doctor for occupational therapy services whether you plan to submit for insurance reimbursement or not. This is to comply with Pennsylvania law.
New Jersey residents need a prescription only if you plan to submit for insurance reimbursement.
Prescription requirements
All prescriptions must include the following:
- Your name and date of birth
- The words “Occupational therapy – Evaluate and treat“
- At least one diagnosis with the appropriate diagnosis code (ICD code)
- Doctor’s signature with the date the prescription was signed
